Adoption Application Form - click here if you want to adopt a dog

We do NOT do on the spot adoptions. Applications must be submitted and vetting/personal references must be checked before an adoption is completed. You must be over 18 years of age to apply. You will not be able to adopt a puppy or dog on the day of your visit if you do not have a verified application. Note: An incomplete form may delay adoption or even result in someone with a completed form adopting the dog you want. Make sure your Vet has permission to pass on your information to Rainbow Animal Rescue. If your vet refuses to talk to us this will delay your application and may result in the dog you want being adopted by another applicant.

Foster Application Form - apply to give a dog a temporary home

Fostering a dog for Rainbow Rescue will free up a kennel allowing us to rescue another dog from the pound. We prefer foster homes to be in the area of Roanoke Rapids and Halifax, NC.
